To apply for the 2010 Guatemala mission

If you are not an existing member please click here to fill out a membership application. You will then receive email instructions on how to proceed.

If you already have an H4H account user name and password you will need to log on then please click here . Once we receive your on line application you will receive email notification of provisional acceptance to the team pending submission of your payment and documents.

2 ways to pay your Membership And Mission Deposit

By Credit Card

Once your application has been accepted you can pay your $25.00 H4H Annual Membership Fee and $500.00 mission deposit by credit card using the donor link OR

By Cheque

Please mail a Cheque for $525.00 (payable to 'Health for Humanity Service Society') to H4H at 15745, 102B Ave, Surrey, BC, V4N 2M1, Canada. This becomes non-refundable once travel expenses have been incurred by H4h.

• Copies of your original certification (RN, MD, FRCS/P Specialist Diplomas)
